Thank you for your interest and willingness to serve for a very worthy cause, Citizens Against Methamphetamine and other addictive substances. Your involvement is vital in reaching our goals.
The following questionaire is designed to place you on a committee that we believe will be most beneficial to CAM. Any information provided to CAM will remain confidential and will be shared only with CAM Board Members.
